CMS 1500 - ML package
CMS-1500 is a standard form for medical claims used by non-institutional providers or suppliers to bill Medicare carriers and durable medical equipment regional carriers (DMERCs). It is also used for billing of some Medicaid State Agencies.
This ML package is trained to automatically detect key information and to extract relevant data from this kind of form. It has over 80 fields covering all types of info that the form can contain.
The model automatically detects data such as:
- Payer Name and Address.
- Patient Address, Telephone, DoB.
- Insurer Name and Address.
- Date of Illness, Claim Information, Hospitalization Date.
- Nature of Illness, Diagnosis Nature, Date of Service, Place of Service.
