ACORD125 - ML package
Acord 125 form known as the Commercial Insurance Application is used to record general client information including things like business location and contact details, business description, prior insurance, and loss history.
This Document UnderstandingTM skill can process ACORD125 forms from 2011 to 2016 and extracts information about the applicant, insurance policy, and other parties involved in the process (up to 68 fields plus items).
The model automatically detects data such as:
- Agency Name, Address, Contact, Customer ID,
- Carrier Name, Policy Number,
- Applicant Information: Name Insured, Mailing Address, Organization Type, GL, SIC, NAICS, FEIN,
- Premises Information: Street, City, ZIP, Interest,
- Prior Carrier Information: Year, Carrier, Policy Number, Premium Amount, Effective and Expiration Date,
