O UB-04, ou CMS-1450, é um formulário de reivindicação padronizado usado por profissionais de saúde para faturar o Medicare, o Medicaid e o seguro privado para serviços institucionais. Inclui detalhes essenciais sobre o paciente, os serviços prestados e os gastos incorridos, simplificando o processo de reembolso para os prestadores de serviços de saúde.

Particularidades

A habilidade é treinada para detectar automaticamente dados como:
