# Business processes and documents troubleshooting

> Diagnose and fix issues with business process mapping in Cartographer: approval prompts, disappearing projects, shallow output, unconfirmed process maps, stale downloads, and generation timing.

## Too many Approve/Reject prompts

**Cause:** Approval mode is set to Cautious, which confirms before nearly every action by design.

**Solution:** Switch to Adaptive or Smart (fewer prompts) or Full access (no prompts), from the chat box or **Settings** → **Basics**. Refer to [Approval mode](local-security-configuration.md#approval-mode) for what each level restricts.

## A project or conversation "disappeared"

**Cause:** Projects are bound to the tenant where they were created — switching organization or tenant hides projects created elsewhere.

**Solution:** Confirm you're logged into the same organization and tenant you used when you created the project.

## Output feels shallow or skips steps

**Cause:** The current model isn't the recommended one, or process-skills didn't match.

**Solution:**
- Check **Settings > Model** — use at least GPT-5.6 Terra, Medium effort.
- Confirm process-skills matched by prompting "Make sure you use process-skills to build your process knowledge."
- If a specific step was skipped, prompt Cartographer to redo it directly.

## Documents not updated despite your confirmation

**Cause:** "Looks good" isn't treated as confirmation.

**Solution:**
- Use the exact phrase "I confirm the process map."
- Check the real captured state in the Checklist panel's FACTS (AS-IS) group.

## Download doesn't reflect my latest edits

**Cause:** The downloaded `.docx` was edited directly.

**Solution:** Use the Refine section workflow or in-product editing instead — see [How refinement works](refining-and-quality-checklist.md#how-refinement-works). Hand-editing the downloaded file doesn't sync back.

## Generation seems stuck

**Cause:** Normal timing — extraction runs around 5 minutes, and PDD generation typically 2–5 minutes, varying by model and effort level.

**Solution:** Give it the full window before assuming a hang.
